I have provided a number of forms on this page for clients. Please feel free to use these forms according to your needs. 

Please fill out the "Preparing For A Therapy Session" form BEFORE you attend EACH session (except the free initial consultation). This form is meant to help you plan the goals of your session. It can be typed online and emailed to me directly before each session (my preferred method).

The "Dysfunctional Thought Records" are used to keep track of your negative thoughts so you can challege them.
